ACKNOWLEDGMENT OF RISK, INJURY, AND OBLIGATIONS

 
Between Phenix Academy and participant /parent or guardians of aged 4-17 years old.
 
I acknowledge that the activity I have voluntarily agreed to undertake is a dangerous activity and that by participating in it, I am exposed to certain risks.
 
Activities include:
 
Striking and grappling
Cardiovascular exercise
Strength and conditioning training
Resistance training
Personal training sessions
Training activities both on and off the premises and other services provided to the participant by The PhenixAcademy.
I (participant/parent or guardian of participant) acknowledge any past/present medical conditions I have/had which may affect or has affected my participation in activities [as described above], will be communicated to the appropriate staff representative prior to undertaking any training.
 
 I acknowledge and understand that whilst participating in such activity:
 
I may be injured physically, emotionally, and/or mentally in the activity
I am doing so entirely at my own risk. Should I sustain injury, Phenix Academy will not be held responsible
My personal property may be lost or damaged
Other persons participating in such activity may cause me to injure or may damage my property
I may cause injury to other persons or damage their property
The condition in which the activity is conducted may vary without warning
I have been notified about the safety precautions for the venue
Any photos taken of me during class may be used for promotional purposes
Whilst participating in on-site training, you accept risk of exposure to viruses and other contagious illnesses and do so knowingly and willingly
By reading this waiver, I agree, acknowledge, and accept the terms laid out above.
